Welcome and thank you for choosing Illinois Retina Associates! In an effort to make your visit as efficient as possible, please take the time to read the following content regarding your upcoming visit.

At Illinois Retina Associates, our goal is to make your visit as pleasant and comfortable as possible. That’s why we take extra time to help you understand your medical condition and give you as much information as you need about your treatment options. If you have any questions, please ask us.

Preparing for Your Visit

A visit to the doctor is rarely convenient. That’s why we try to schedule all necessary examinations and tests during your visit. You will see a nurse or ophthalmic technician, your doctor and other staff, if needed. Understandably, this takes time – sometimes two to three hours, or even longer if your doctor feels that immediate treatment is necessary. We kindly ask for your patience.

We also suggest that you have a family member or friend accompany you on your first visit since you will receive important information about your condition and its treatment. Having someone with you will help you retain and remember more. During this visit, we will also dilate your eyes, so we strongly advise that you not drive.

In fact, for your safety, plan on having a friend or family member accompany you to any appointment that involves a procedure or treatment of any kind so that you do not have to drive.

Below are a list of other items regarding your visit to Illinois Retina:
1. Prepare for Your Office Visit.

In addition to completing the necessary forms, please bring the following documents and information to your first visit:

  • Driver’s license or other valid government-issued photo identification
  • Insurance card and any co-payment due for visit
  • Referral or consultation form if applicable
  • Any pertinent medical history such as lab results
  • A list of all medications you are taking

Please note, we will dilate your eyes during your visit. So we strongly advise you not to drive. For your safety, please bring someone with you if possible.

2. Fill out Patient Forms.

Print your forms, fill them out, and bring them with you to your first appointment.

3. Get Directions to Our Office.

Visit our Locations page to find a map to the Illinois Retina Associates office where you are scheduled to see your physician. You can then get personalized driving directions right to our front door!

4. Arrive Early for Your Appointment.

Please arrive at least 30 minutes early if you have not yet filled out the new patient forms. If you have completed forms with you, you can check in with our reception desk just 5 minutes before your scheduled appointment.
