To set-up your patient portal account, please have the following information available:
- Your personalized invitation link (email or text) or a printed invitation from our office with a unique identification code
- Patient Name and Date of Birth
- Our Practice ID: ILLRET
- Your 5-Digit Zip Code
For optimal performance, My Patient Visit should be accessed via Google Chrome.
